The author’s journey from developing services for vulnerable children in the UK to addressing the most challenging needs of communities in low-income countries spans 20 years. This personal experience is contrasted to therapeutic advances in the field, and emerging evidence. It is argued that low and middle-income countries require different models rather than the simple adaptation of western services for hard-to-reach groups.
