ABSTRACT

This chapter presents a case study on man-made disasters caused by a structural failure of a bridge. Having policies and procedures is important, but it is even more important for administrators to remember that there needs to be a comprehensive process established to ensure that those ordinances are followed when projects are being formulated and constructed. There was no apparent mechanism in place to ensure that building codes were being followed during the design or implementation stage of construction. Government administrators should ensure that a mechanism is in place to evaluate critical infrastructure when modifications are being made to an existing structure. During the modifications, the renovation plans should have included an evaluation by engineers concerning whether the initial design could support the increased weight from the modifications as well as the increased traffic the bridge was now incurring on a daily basis.
