ABSTRACT

This chapter presents an analysis of the injury data that was collected via understanding and preventing led outdoor accidents data system (UPLOADS) by 31 organizations during a one-year period. The findings from the analysis have several important implications for organizations wishing to understand, and prevent, incidents their own incidents. The chapter describes a step-by-step process for analyzing the findings from an incident reporting system underpinned by systems thinking. To illustrate how to report and interpret the findings from such an analysis, an analysis of injury data collected via UPLOADS is presented. The purpose of this process is to produce an analysis report to inform the development of incident prevention strategies. It is assumed that the incident reports have already been analyzed using an accident analysis method based on Accimap, with an associated contributory factor classification. A well-designed and effectively implemented incident reporting system is a significant resource for safety monitoring and improvement activities.
