Transition from one setting/school to another

In order to ensure a smooth transfer a number of considerations need to be made, as the pupil makes the transition from one setting to another, be it from pre-school to primary school or primary to secondary school. However, all the following points may not need to be acted upon, this will depend upon the severity of the pupil’s condition.
