ABSTRACT

Oliver has read the report on likely causes of a failure of scaffolding during building repairs. He re-reads it for the key points and then makes a list of implications for the company. He also lists a series of questions for the reports’ authors: he needs clarification on some points and more information on others. He calls them. Later he meets the Health and Safety Manager, Nancy. At the end of the meeting Nancy says: ‘I can see you’re on top of this, Oliver. Get back to me with some suggestions to deal with safety issues. I realise that the contractors were largely responsible but the accident doesn’t sit well with company values. We need to look more closely at our systems and processes.’
