In Chapter  1, we focused a spotlight on the global financial crisis that unfolded between 2007 and 2009, the aftershocks of which are still with us today. We suggested failures of leadership were responsible for much of that debacle. Loosened regulatory restraints led to an unholy collusion between banks and ratings agencies, which led in turn to massive amounts of sub-prime loans being packaged into collateralized debt obligations (CDOs) and then into derivatives.
