ABSTRACT

The Jewish Home and Hospital for Aged is providing a range of community-based services at its campuses in Manhattan and the Bronx to as many or more people as receive inpatient services—that means more than 1,300 people in communities throughout those two boroughs. Clients begin to view the nursing home not as a place to be feared, but as a place that encourages their independence and provides resources to assist them in remaining independent. Every program interfaces with the larger service delivery system, including hospital admitting departments and discharge planners; senior housing providers and landlords; home care providers, public and private; and senior centers and day care programs. Students can be involved in developing nursing home-based community programs by realizing that while there are nursing homes that continue to provide inadequate care, there are many high-quality nursing homes with a commitment to residents, to communities and to learning.
