ABSTRACT

This chapter introduces and discusses an organization (Paradigm Health Corporation, Concord, CA) that, over the past several years, has developed an innovative model of managed care for the treatment of severely brain-injured survivors. This model has integrated outcome-based service delivery with shared-risk reimbursement. There is increasingly widespread recognition that the clinical and technical ability to provide treatment for brain and other catastrophic injuries, as well as other health care problems, is inextricably linked with the organizational and economic structures through which such care is delivered. This “economic” influence on the dimensions of care delivery offers the rationale for a “systems” presentation.
