ABSTRACT

This article reviews three models of intensive case management developed and tested in Monroe County (Rochester), New York: the Centralized Individual model, the Neighborhood Team, and the Home Health Care Team. The three models are described and contrasted, and the role of the social worker in each is discussed. The models differed especially regarding client assessment and reassessment, direct service provision, and crisis intervention. The cost-effectiveness of the two Team models is highlighted, particularly for dementia and terminal patients. We conclude with a discussion of the relationship between different levels of case management intensity and optimal targeting.
