ABSTRACT

This chapter summarizes the overall process of questionnaire design, administration, and use. It explores the purpose for the questionnaire, what one want to learn, and how will the results be used. The answers to these questions should guide the entire process. With the purpose in mind, the questionnaire committee needs to brainstorm the desired concepts to be measured. A literature review of content may be needed to understand what information needs to be captured. Disseminate findings of the literature review and create an awareness of how to develop effective questions to meet the purpose of the questionnaire. To collect the data from the questionnaire, develop strategies to ensure 100" participation of staff, students, and parents. For students, the questionnaire can be administered during the school day during a time when all the students are working at a computer.
