ABSTRACT

This chapter explores the multiplicity of factors which have influenced the development of care management for older people with dementia thereby providing contextual information for the description and analysis of the Lewisham Case Management Scheme. It provides information about the development of care management arrangements to co-ordinate community-based care for older people with mental health problems and in particular those with dementia. In the USA, Eggert and colleagues reviewed three models of intensive case management, also incorporating a health dimension a 'Centralized Individual' model, a 'Neighborhood Team' model, and a 'Home Health Care Team' model. Care management has its origins in North America. It developed as part of the move in the balance of care away from institutional to community-based provision. Of particular significance in this context was the family of 'Channeling Projects' commissioned by the federal government of the United States in the 1980s.
