ABSTRACT

This chapter focuses on managing the event and preventing the same thing from happening again. It identifies the risks in healthcare and takes the Advanced Incident Management System (AIMS) as a template. The chapter also focuses on evaluating, analyzing and treating the risks in healthcare and holds the structured approach to Root Cause Analysis (RCA) developed by James Bagian as a template. It describes that the medical defence organizations in Australia have been made aware of more than ten such cases each year, but their usual practice is not to disseminate this information. The chapter considers that the facility should also exist for healthcare professionals to submit reports anonymously for inclusion in a database controlled by the professional body responsible for their area of practice such as a specialist college. The severity assessment code (SAC) is used to determine who should be notified, whether more detailed information should be obtained, and whether a RCA should be done.
