ABSTRACT
Safety recommendations are the end-point of an accident investigation, but many are not acted upon. Recommendations are more likely to be effective if they address underlying organizational or management deficiencies, rather than individual failings or local conditions. However, the difficulty has been, firstly, to identify such underlying deficiencies, and, secondly, to do something about them. The Theory of Constraints is a change methodology, designed to address these matters.
