ABSTRACT

This chapter provides a step-by-step guide to designing incident prevention strategies using a systems thinking–based approach. The process involves stakeholders from across the levels of a system working together to identify incident prevention strategies. The application of the process is illustrated by describing the incident prevention strategies that were developed by stakeholders based on an analysis of injury data collected via understanding and preventing led outdoor accidents data system. Systems thinking is based on the view that incidents are caused by multiple, interacting, contributory factors. To understand and prevent incidents, organizations need to ensure that this view is embedded in the way they analyze incident data, and in the way they design incident prevention strategies. The systems thinking approach suggests that organizations need to address incidents holistically, rather than just attempting to 'fix' broken parts of the system. The chapter describes participatory design process for translating Accimap analyses of incidents into strategies that align with J. Rasmussen's risk management framework.
