ABSTRACT

This chapter describes how to adapt Accimap for use within an incident reporting system. The process involves two tasks: adapting the labels on the levels of the Accimap framework to better describe the structure of the system in question; and developing a domain-specific classification scheme describing the contributory factors found at each level of the Accimap framework. The chapter describes how to undertake these tasks by drawing on multiple data sources, including workshops with subject matter experts, the literature, case studies of fatal incidents, and incident reports. Accimap represents the system under analysis as a hierarchy comprised of multiple levels. The adaptation of Accimap for use within understanding and preventing led outdoor accidents data system is used to illustrate the process. Adapting Accimap for use within an incident reporting system requires the development of a classification scheme which comprehensively describes the contributory factors at each level of the Accimap framework.
