ABSTRACT
A number of common features can be identified: • The last stages of both processes, suggesting action to prevent accidents, are identical. • Good accident investigations are rarely restricted to the one set of (possibly unique) circumstances which
led to the outcome under investigation. A good investigation will consider similar circumstances which could occur elsewhere in the organisation, and include these in the investigation remit. In other words, a good investigation will include an element of risk assessment.
