ABSTRACT

This chapter addresses transition plans, as well as strategies and practices that have resulted in the development of collaborative models and operational partnerships among high school personnel, college disability service staff, other higher education professionals, and the adult service system. It features the National Technical Assistance Center on Transition's (NTACT) Transition Fair Toolkit, which provides a comprehensive set of materials for planning or participating in a transition fair. The process should begin with identification of student preferences, interests, and adult life goals. Students can utilize technology-based applications while in high school to enhance their skills academically and socially, as well as to increase independence. The chapter highlights evidence-based methods, and practices that have helped improve the chances of college and career readiness for students with disabilities, such as utilizing technology and engaging students and families in the transition process.
