ABSTRACT
The following notes are taken, verbatim, from the patients “Clinical Folder,” maintained while he was in the neuropsychiatry hospital. They are presented in order to give a picture of how this patient appeared to various professional personnel at different times. The dates of other events, such as the psychological testing, the initiation and termination of insulin coma therapy, etc. have been inserted. Attention is directed to the various diagnostic labels given this patient during his relatively short hospitalization. Actual dates, doctor’s initials, and other possibly identifying data have been changed, but the relative chronology of events has been preserved.
