Imagine that you’ve just carried out a comprehensive assessment of a 75-year-old woman called Mrs Perkins. During your assessment Mrs Perkins tells you that she is suffering with constipation. You therefore explore this problem in more detail, finding out, for example, how long it is since she last opened her bowels, what remedies she’s tried, and whether she’s suffered from constipation before.
