ABSTRACT

A wound assessment is an important/legal piece of documentation that should be included in a patient’s notes if a wound is present. It enables you to ascertain the original wound size and description of wound appearance; it is also used continuously to monitor if there has been any deterioration or improvement. There must be a systematic approach to wound assessment, which starts by classifying the wound and information gathering using various assessment tools and investigations, identifying treatment objectives, deciding on the appropriate interventions and documenting the findings (Collier 2003).
