In any case the special services and pharmaceutical techniques among many other considerations make close and frequent consultations with pharmacy an important part of the delivery of nutritional support to the patient. Decentralized team operates by providing a consultative service to aid the primary physician and ward nurses, and the clinical pharmacist to formulate and deliver total parenteral nutrition (TPN) in a safe and effective way to the patient. The main advantage of this system is that patients remain under the care of their primary physician and TPN is thus given in the context of total care. With a centralized team, patients requiring parenteral nutrition are taken over by the core team members and their assistants. The chief advantage of this system is the easy enforcement of high standards of care in regard to the delivery of TPN. In a small hospital with limited expertise this approach is economical in terms of resources.
