A failure mode is defined as the way(s) that a system or subsystem could fail to perform its intended function(s). Effects, on the other hand, are the consequences that will take place as a result of a failure mode. In order for this activity to be effective, we must specify the who, what, where, when, and scope of potential problems. In this first step, suppose we have identified a sole supplier of a product as a vulnerable area or area at risk. The answer to what in this case might be the inability of the supplier to ship specific parts or materials without adequate advanced notification. Since we have no backup supplier, a delayed shipment of parts or a materials shortage could last for some time.
