ABSTRACT
Effectively documenting people’s communication provides a way of promoting
consistency between practitioners, and helps new practitioners to quickly gain
a shared understanding of how to support someone. Furthermore, collating
information and reviewing existing documents creates an opportunity for people
to share experiences and knowledge about supporting an individual, thereby
enhancing each other’s understanding of that person and how best to support them.
