ABSTRACT

The task of projecting the future of health information technology (IT) in the long-term/post-acute care (LTPAC) provider market becomes harder when recognizing the dynamic nature of the industry. A prime example of the changing nature of the healthcare ecosystem is evident in efforts to define the aim of healthcare. To get a sense of the future of health IT in the LTPAC provider market, it is important to explore significant trends influencing the worlds of digital health and the LTPAC industry. While by no means an exhaustive list of the array of forces affecting these areas, the following chapter considers two significant trends: Person-centered care models and Longitudinal health records. This chapter notes that LTPAC can be a highly valued member of person-centered models of care in the US healthcare system. Longitudinal health records are a natural fit for the care delivery system provided for persons in LTPAC settings.
