ABSTRACT

In this chapter, we discuss eight cases (introduced in Chapter 1) wherein poor maintenance has been cited as one of the causal factors. The original accident reports were obtained either from the National Transportation Safety Board (www.ntsb.gov) or Air Accidents Investigation Branch of the U.K. Civil Aviation Administration (www.aaib.dtlr.gov.uk). Each case is analyzed to illustrate the causal chain that led to the particular accident, and specific risks involved in each accident are discussed.
