ABSTRACT
Many aspects of errors have been examined and numerous accidents cited. One accident will illustrate several additional points, describe challenges that investigators could face, and demonstrate the data gathering and analysis processes used to identify critical errors and the antecedents that led to the errors, and recommendations to remediate system deficiencies.
