ABSTRACT
This is the story of how a chance meeting on a commercial aircraft led to the inclusion of Human Factors analysis in Canada's longest running inquest, the Pediatric Cardiac Surgery Inquiry (PCSI). In addition, a summary of some of the results of that analysis is provided.
