ABSTRACT
I have very carefully entitled this chapter simply ‘Error’. Many people talk about and write about ‘pilot error’ as if it were something special. It is not. The errors pilots make are no different in their nature to those made by human beings in other walks of life. It is merely their context that is unique and unfortunately their consequences are potentially more severe.
