All decisions have unintended consequences, and even sometimes the complete opposite of what was intended. In retrospect, the failure seems inevitable. The problem escalates as the water slowly heats up. The project starts with bright promises, no one probes the risks, ambiguity lurks in the specifications, suppliers provide what is available instead of what is needed, indecision accumulates and then one day – far too late in the day – the project is cancelled.
