ABSTRACT

This chapter presents a case study of a 6-year-old child who has a missed fracture of the distal radius. Torus or 'buckle' fractures are sometimes missed, as radiographic findings may be subtle. Most Emergency Departments (ED) have several safeguards already in place regarding radiographs with abnormal findings. Firstly, radiographers will place the words 'red dot' on immediately abnormal radiographs. This serves as an additional reminder to the attending practitioner that there is an abnormality present. With electronic systems, reporting radiologists and radiographers can check that appropriate follow-up or actions have been taken. In those that have been 'missed' or in which there is a query, a report is usually communicated to the senior ED team for review and action as appropriate. It is therefore essential that the most up-to-date demographics and contact telephone numbers are recorded for all patients attending the ED.
