ABSTRACT

Through this book I want to share the knowledge and experiences I have been fortunate enough to collect over the past 15 years and offer healthcare professionals concrete suggestions on how they can avoid adverse events. Using reallife case stories of adverse events, I describe behaviour that impairs patient safety. The healthcare professionals involved were mostly not aware of this unsafe behaviour or the consequences this behaviour could have. In the reflection on each case I try to understand the behaviour and suggest alternatives that can help the reader improve patient safety. The ‘lessons’ from the book can be summarised as follows:
