ABSTRACT
The aerospace industry has several tools that are based on the above hypothesis, that a mishap is never a result of a single event. It is always a combination of at least two events and often multiple events. The tools are institutionalized under an umbrella called System Safety. By no means is system safety science all-inclusive, but it does provide the structure and tools for preventing adverse events.
