ABSTRACT
Unit I: Steps 4, 5, and 6, Introduction Purpose In Chapter 2 we went over the information that covers the upper portion of the claim form, which consists of the patient name and address along with the insurance information including the address to which claims are sent. Chapter 3 will cover the next 3 steps, which involve the bottom portion of the CMS-1500 claim form (see Figure 3.1).
