ABSTRACT
INTRODUCTION To this day, history taking forms the basis of medical practice worldwide. After all, in the majority of cases, the correct diagnosis can be made from the history alone. Viewed simplistically, the medical history is an exercise in data gathering. This dataset can not only help formulate diagnoses but also ascertain possible causes, assess the impact of illness on patients and guide more focused examination, investigation and subsequent management.
