Hair restoration as a treatment to hide scars on the scalp was used by A. J. Barsky in 1950 with the implantation of small grafts and by E. S. Lamont in 1957 by using flap techniques. Hair restoration surgery is performed under local anesthesia. The design of the hair transplant is most crucial for the outcome of the procedure and for patient satisfaction. The donor hair supply for one single session has to be taken into consideration as a first step. The patient needs to understand that hair restoration surgery is a redistribution of hair and that the hair can never be restored the way it was in his or her teen years. In primary cicatricial alopecia, hair restoration surgery is an option for burnt-out stages, especially for lymphocytic primary cicatricial alopecia. The consultation process is the key to a good relationship between the physician and the patient, to a successful hair restoration procedure, and to patient satisfaction.
