ABSTRACT

This chapter covers the basic aspects of teams and how team actions affect both HAZOP and FMEA results. The information in this chapter does not represent an exhaustive examination of teams but does cover issues related to both methodologies. To achieve best results, a risk analysis (HAZOP, FMEA, FTA, etc.) must be written by a team. This is because a risk analysis should act as a catalyst to stimulate interchanges of ideas among the groups affected (Stamatis 1991). A typical view is shown in Figure 8.1.
