All of the existing methods for reconstruction of the pharynx and cervical esophagus following total laryngopharyngectomy for advanced cancer are short of ideal, some of them considerably so. A new pedicle flap technique that accomplishes the major part of the reconstruction at the time of the resection has been developed, and reduces some of the disadvantages to an acceptable minimum.1
