Human Factors of Transition of Care

Continuity of care-what exactly does it mean? What do human factors have to do with achieving continuity of care? Let us review a case, refer to fig­ ure 15-1, from a 2002 New York Times article dis­ cussing continuity of care in a New York hospital that implemented 8-to 12-hr shifts for physicians with regard to sleep error issues (Zuger, 2002). This case raises several issues related to continuity of care.
