Bacterialadhesiontohumanepithelialcellsoftheurinarytractisanimportantstepinthe establishmentofurinarytractinfectionsandchildhooddiarrhea.StrainsofEscherichiacolithat causepyelonephritisattachtoGal(1,4)Galstructuresfoundaspartofthemembraneglobosides (42,43).Thiscarbohydratestructureoccursaspartofthepk,P,andP1bloodgroupantigens. Onestudy(43)showedthatgirlswhoseRBCtypedasP1positivehadanincreasedriskfor recurringpyelonephritis.Thepresenceofthefimbriaethatcauseattachmentappearstobe inducibleandisduetoa92-kbplasmidfoundintheinfectiousstrains(44).Screeningforthese diseasesusingRBCwithhigh-densityP1antigenmaybeausefulclinicaltest.
