ABSTRACT
As described in Chapter 1, organizations have dual goals. One goal, as previously illustrated in Figure 1.2, is to enhance “success space.” Such enhancement efforts often include a healthy dose of improving the efficiency and productivity of various work processes. Another parallel goal is effectively managing “failure space.” Here, the challenge is to prevent, or at least significantly negate, the adverse effects or consequences of an unwanted incident.
