You need to file two further forms: ❍ form 4.9, Notice of Intention to Appear on Petition;
and ❍ form 4.10, List of Persons Intending to Appear on
the Hearing of the Petition.
Both forms appear in a tear-off version at the end of the Insolvency Service’s leaflet Dealing With Debt: How to Wind Up a Company That Owes You Money.
