ABSTRACT
We need connectors. Connectors can do wonderful things for the right patients. Nearly 75 percent of all care costs come from patients with chronic conditions. Eighty percent of the care costs for those patients comes from patients with multiple health conditions — co-morbidities. Patients with co-morbidities tend to have separate doctors for each diagnosis because care delivery in America is now very specialized.
