ELIMINATING QUALITY DEFECTS continues to be a major issue on the shop floor. While few manufacturing operations today have defect rates above 2 or 3 percent, those with rates below 1 percent are probably focused on defects that have become chronic. Unfortunately, chronic problems-whether Losses of quality, productivity, or both-just don't seem to go away.
