ABSTRACT

He had been born by emergency caesarian section in response to fetal distress and had an Apgar score of 4 on delivery. Seizure activity developed following three prolonged febrile convulsions early in his first year of life and continued throughout childhood, despite removal of a damaged left temporal lobe at the age of 3 years. He was aggressive before the clusters of seizures, which were always followed by complex automatisms. His parents found his constant wandering particularly difficult to cope with. Treatment on referral was with phenytoin monotherapy. He had received carbamazepine, valproate, phenobarbital and bromide in the past.
