ABSTRACT

Neurological complications occur in over half of all critically ill patients, and these may affect their outcome as well as their length of stay (Vespa, 1998). Following any neurological insult, nursing assessment is vital in order to measure progression or deterioration. Most nurses should be familiar with a neurological assessment chart, which should include four distinct sections:

1 The Glasgow coma scale (GCS) 2 Pupil reaction 3 Vital signs 4 Limb responses.
