ABSTRACT

Following initial treatment with heparin (either UFH or LMWH), prevention of recurrence can be achieved by pharmacological means. Warfarin is traditionally used for long-term treatment; for this purpose, one aims to maintain an INR of 2.0-3.0.31 Unfortunately, due to the already mentioned unpredictable pharmacokinetics of coumarin derivatives, this treatment is not compatible with invasive procedures and surgical interventions.
