The day before I started work on this chapter, I was under considerable pressure from a `traditional service' to agree to abandon the expectation that a transfer summary would be supplied when our patients leave our service. Our early intervention in psychosis team works with patients for a three-year period and some of them need further help from mental health services. I was told that the team, to whom some of our patients were moving on, did not need `all that stuff'. All they needed was a `diagnosis' and a risk and care plan.
