Attention concerning the safety of the care provided to patients has increased substantially in recent years, with hospitals and other health care institutions now required to have a safety management system in place. Registering and analysing incidents, where incidents are defi ned as ‘everything unintended’, represent a signifi cant element of such systems, while the analysing of incidents can play an important role in the quality and safety of the care provided to patients. Experience with similar reporting systems in other sectors, such as the aviation sector, has been good.
