Increasing Social Support to Manage Chronic Illness

Families dealing with chronic illness often experience a depletion of emotional and practical resources overtime. As resources dwindle, secondary illness related to the chronic illness (e.g., depression) can be found in other family member as well as the primary patient. The objective of this intervention is to remind the family that they have social resources that can be accessed, and to facilitate the connection to these resources.
