Surgical treatment of endometriosis
The extent of surgery is dependent on the preoperative symptoms, the severity of disease, the wishes of the patient and the need for informed consent. Most clinicians use the revised American Fertility Society (AFS) scoring system for endometriosis, comprising four groups: minimal (stage I), mild (stage II), moderate (stage III) and severe (stage IV), according to the operative findings1. Various treatment modalities are available for use at laparoscopic surgery. These include: laser, scissors with monopolar electrocautery and bipolar coagulation, harmonic scalpel. All allow resection, cauterisation and vaporisation of endometriosis.
